Report an Incident or Concerning Behavior

Non-Academic Misconduct

Non-academic violations are behaviors that include, but are not limited to, alcohol, disorderly conduct, drugs, hazing, harassment, physical violence, or other violations of university policies, regulations, or rules.

Academic Integrity Violations

Academic integrity violations are behaviors that include, but are not limited to, cheating, plagiarism or falsifying official records related to the University. General classroom misbehavior is considered a non-academic violation. Instructors should use the Report of an Academic Integrity Violation (RAIV) form to address their concerns with the student. Instructors should submit all relevant materials and accompanying materials using this form.

NOTE: Starting Spring 2023, we are no longer requiring the use of the paper RAIV form. Instructors should still meet with their students to review the allegations and supporting materials, then move forward with reporting the incident to the Office of Student Conduct through our online form, avaliable at go.ncsu.edu/RAIV

Report Relationship or Interpersonal Violence, or Sexual Misconduct

Interpersonal violence includes dating violence, domestic violence, sexual harassment, sexual misconduct, or stalking behaviors. The Office of Student Conduct works with the Office for Institutional Equity and Diversity to address interpersonal violence concerns.

Report Concerning, Worrisome, or Threatening Behaviors

Faculty, staff, students, and guests are encouraged to report behavior that they perceive as concerning, worrisome, or threatening. For more information, please visit the NC State CARES page and review information on when to refer a student.

Report Community Climate Concerns and Bias Impact

The OIED Impact Response Team reviews reports of bias-related events, incidents, behavior or actions and supports restorative and educational responses to address the individual, collective, and institutional impacts. Written Student Complaint Process

NC State is committed to excellence in academics and support services. When students believe NC State is not meeting its responsibilities under the law or our own policies and procedures, they are encouraged to know and work with us to resolve the issues. The University has designated resources to help maintain our professional standards across campus and to provide appropriate responses to complaints and concerns.
