10 Day Notice of a Disciplinary Hearing

This form is for Respondents who wish to expedite their Administrative Hearing or Conduct Board Hearing and waive the ten-day notice of charges. 

Academic Misconduct Hearing Process Waiver

Respondents who wish to have their pending academic misconduct matter resolved through a Disciplinary Conference and waive a formal hearing process may do so by completing this form.

Appeal Form

This form allows for students to appeal the outcome of any disciplinary process facilitated by the Office of Student Conduct or University Housing, with the exception of Title IX/DHR related cases.

Dean Certification & Disciplinary Records Request Form

This form is for current and former NC State students who are requesting to have a copy of their student disciplinary records or have it released to third-parties by The Office of Student Conduct (OSC)

Five Day Notice of a Disciplinary Conference

This form is for Respondents who wish to expedite their disciplinary conference and waive the required five-day notice. This form does not apply to cases where a student is at risk for suspension or expulsion, as they must go through a hearing process.

Interpersonal Violence and Sexual Misconduct Hearing Appeal Form

This form is for appeals related to Interpersonal Violence and Sexual Misconduct cases.

Multiple Student Hearing Consent Form

This form is for Respondents who wish to participate in a hearing with other involved individuals.

Notice of Attorney or Non-Attorney Advocate Form

In order for a licensed attorney or another advocate to participate in the hearing process, a student or student organization must provide a completed form to the Office of Student Conduct.

This online form must be submitted separately by the Student and the Attorney/Non-Attorney Advocate at least three (3) calendar days prior to the scheduled disciplinary proceeding.

Notice of Student Conduct Board Members

This form provides Respondents with the opportunity to challenge a Board member scheduled for a Conduct Board Hearing for cause.

Petition for Transcript Notation Removal

This online form allows for students to request a removal of the suspension notation on their transcript.

Release of Information

This form allows the Office of Student Conduct to release information to identified individuals, such as a Respondent’s parent. This is primarily used if the Respondent would like their parent to attend their disciplinary meeting or hearing. 

Request for Sanction Deferment and/or Extension of Time to Appeal

This form is to request a deferment of sanctions pending the appeal process and/or extension of time to submit an appeal.
